Job Description

The RN Case Manager plays a vital role in managing chronic and complex medical needs for members across the continuum of care. This position supports the practitioner-patient relationship, promotes adherence to preventive care, and assists members in self-managing their chronic conditions. The role involves comprehensive care coordination, clinical assessments, developing individualized SMART care plans, member education, and facilitating multidisciplinary team collaboration. The RN Case Manager ensures compliance with Medicaid contracts, organizational policies, and Patient-Centered Medical Home (PCMH) standards, driving improved health outcomes and reducing gaps in care.

Responsibilities

Conduct comprehensive care coordination and clinical assessments.Develop individualized SMART care plans for members.Educate members on self-management of chronic conditions.Facilitate multidisciplinary team collaboration.Ensure compliance with Medicaid contracts and PCMH standards.Perform risk stratification and support hospital discharge planning.Conduct outreach to high-risk or high-utilization members.Track clinical outcomes and identify barriers to treatment adherence.Document according to quality, regulatory, and accreditation standards.Participate in team huddles and escalate questionable treatment plans to medical leadership.

Essential Skills

Active RN licensure in Florida.3–5 years clinical experience in chronic disease management or complex case management.Experience in managed care or health plan settings (Medicaid/Medicare regulatory familiarity).Ability to develop and execute SMART care plans.High-level communication skills for member education, team huddles, and multidisciplinary coordination.Experience with discharge planning, care plan creation, and resource referral.

Additional Skills & Qualifications

Certified Case Manager (CCM) credential preferred.Pediatric experience, especially in PICU or complex pediatric care.Motivational interviewing experience.Experience with utilization management processes (authorizations, appeals).Proficiency in Electronic Health Record (EHR) systems, particularly EPIC.

Work Environment

This role is part of a mission-driven team dedicated to improving maternal and child health outcomes across Florida. It offers flexibility, training opportunities, and the chance to make a measurable impact on community health. The work environment is supportive and values professional growth and collaboration. The position is hybrid, with occasional onsite requirements in Sunrise, FL. The schedule is Monday to Friday, 8:30 AM to 5:00 PM, with flexible start times and a 30-minute lunch break. Orientation and training sessions are conducted onsite, with computer pickup and EPIC training included.

This is a Contract to Hire position based out of Fort Lauderdale, FL.

Pay and Benefits

The pay range for this position is $38.00 - $42.00/hr.
