Case Manager (Mon-Fri 9a-5p) Buffalo, NY (http://maps.google.com/maps?q=486+N.+Legion+Drive+Buffalo+NY+USA+14210) Apply Description + Responsible for the case management of assigned individuals. + Makes referrals and assists with the coordination of treatment services. + Helps individuals identify community agencies, housing opportunities, resources, and supports. + Provides assistance to residents with financial linkages and resource management + Coordinates educational and/or vocational skill development. + Helps each individual develop an informal support system, including natural supports and those requested by the individual. + Ensure smooth transitions and minimizes fragmentation of services. + Monitors, tracks, and provides support to the individual. + Organizes and maintains case records. + Assists with intake and admission. + Serves on various agency committees. + Assists in the implementation of the agency’s policies and procedures. + Must abide by the ethical guidelines of the field and any licensing authorities. + Facilitates the resident government’s program guidelines. Requirements + Enrollment in or graduate of a college degree program in a Human Services related field, with at least 6 months’ experience (may include internship experience). + ?Must possess a valid New York State Driver’s license.
