Salary Range $23.69 - $25.00 Hourly Position Type Full Time Job Shift Any Education Level 2 Year Degree Travel Percentage Road Warrior Description Position Summary: “This position is assigned in direct support of the Medication Assisted Treatment (MAT) Mobile program and will contribute to its day-to-day operations, client engagement, and service delivery”. Person in this position will work in collaboration with a mental health counselor to provide mental health services to patients/clients of WestCare who have been diagnosed with a substance abuse disorder, a mental illness or both. Essential Job Functions include those listed below. Other duties may be assigned as needed. Essential Job Functions: + Assess resource needs of patients. + Educate and refer patients to community agencies and businesses that appropriately address the patients’ needs for food, shelter, clothing, employment, transportation, etc. + Assist patients with establishing or re-establishing support systems within their community. + Act as advocate on behalf of the patient to improve access to needed services. + Participate in clinical staffing decisions regarding the patients’ current strengths and resources and their need for additional resources. + Complete documentation of services on provided forms in a timely manner. + Provides input into the patient’s treatment needs and disposition. + Assists other WestCare staff as needed in a collaborative manner to promote cooperation and portray a professional image to the community. + Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation Qualifications Education: + A minimum of a bachelor’s degree in social work; or a Bachelor’s degree with a social work major; or working towards a degree in social work; or comparable years of qualifying experience Experience and Competencies: + Ability to work well in a team environment. + Ability to exercise good judgment and discretion. + Ability to operate a computer and complete documents in Microsoft Word program formats or the ability to learn. + Ability to respond as needed to crisis situations in an efficient manner. + Ability to complete work tasks within scheduled work hours. + Excellent verbal and written communication skills. + Willing to seek out new work projects and contribute ideas and efforts to the constant improvement of WestCare services.
