Case Manager
Job Title: Care Transitions Planning ManagerJob Description

The Care Transitions Planning Manager plays a crucial role in managing care transitions throughout the acute care patient experience. The position collaborates with physicians, utilization managers, medical social workers, and bedside teams to ensure timely patient progression and transition to appropriate care levels, preventing unnecessary admissions or readmissions. The role involves effective communication and resource coordination to facilitate care across the continuum, aiming to achieve optimal health, access to care, and appropriate resource utilization while respecting patient self-determination.

ResponsibilitiesCollaborate with patients, families, physicians, and the interdisciplinary team to ensure timely care transitions.Work with nursing management, quality, ancillary services, third-party payers, review agencies, claims and finance departments, medical directors, and contracted providers to coordinate care.Manage complex clinical and social situations in the Emergency Department to avoid unnecessary admissions.Facilitate communication and coordination of care across the continuum.Ensure optimal health outcomes, access to care, and appropriate resource utilization.Essential SkillsCase managementCare managementPrior authorizationRegistered Nurse (RN) licensure in California2 years of experience in Case ManagementExperience in Utilization Management and working with attending physiciansAdditional Skills & QualificationsExperience in utilization reviewKnowledge of admission criteriaExperience in care coordination and discharge planningWork Environment

This position involves a 4-day workweek with shifts from 8:00 AM to 6:30 PM, rotating weekends, and no on-call responsibilities. The work environment includes collaboration with a diverse team of healthcare professionals and the use of advanced care coordination technologies.

Job Type & Location

This is a Contract position based out of San Francisco, CA.

Pay and Benefits

The pay range for this position is $50.00 - $60.00/hr.
