This role plays a critical part in helping members navigate complex health needs by coordinating care, assessing clinical situations, and creating individualized plans that support positive outcomes. Your clinical expertise and advocacy will drive improved quality of care, appropriate resource utilization, and overall member well‑being.What You’ll Do
• Serve as a member of a multidisciplinary team, coordinating care, resources, and services to achieve optimal clinical and resource outcomes
• Perform comprehensive clinical assessments and collaborate with members, families, providers, and internal teams on customized treatment or intervention plans
• Develop member‑centric care plans that identify needs, barriers, and strategies to meet goals
• Leverage community resources and funding sources to support care plan development
• Monitor members through regular contact, track progress toward goals, and adjust plans as needed based on acuity
• Evaluate and support care throughout the full care continuum, including hospitals, rehab, skilled nursing, and home care
• Educate members on proactive health strategies to encourage improved outcomes
• Fully document assessments, plans, interventions, and outcomes
• Conduct outreach to high‑risk, high‑cost, or high‑utilization members
• Review treatment plans and assess benefit eligibility, benefit exceptions, or flex benefit options
• Assess medical necessity and service appropriateness based on department standards
• Develop policies, procedures, training, or education as needed for internal or external partnersWhat We’re Looking For
• A current, up‑to‑date resume
• RN, RD, or LCSW with 3 years of clinical and/or case management experience (MSW with 5 years may be considered for some roles)
• For Behavioral Health roles: other applicable licensure with at least 3 years of clinical experience may be accepted
• Valid and active NC or compact multi‑state clinical license
• Specialty certifications (such as CCM, CDCES) may be required; must obtain within 2 years if applicable
