Case Manager - East Boston

Supporting families experiencing homelessness in one of the region's largest shelter and stabilization programs, this role plays a key part in helping parents and children work toward safety, stability, and long‑term independence.

Requirements

Bachelor's degree in Human or Social Services2+ years of case management experienceBackground working with vulnerable or high‑needs populations

Job Description

Provide intensive case management, assessment, counseling, and advocacy for families in shelterBuild goal‑oriented, supportive relationships with families from intake through stabilizationCollaborate with Housing Specialists, Behavioral Health Clinicians, and Family PartnersSupport families in developing rehousing and stabilization plans within a trauma‑informed frameworkMaintain accurate case files and documentation for internal and external reportingDeliver crisis intervention in partnership with behavioral health and leadership teamsServe as a positive role model and advocate for families' needs across systems

Schedule

Full‑time, 40 hours per weekMonday-Friday: 8am-4pm or 9am-5pm, with one late shift (10am-6pm or 11am-7pm)On‑call rotation: one weekend per month + one weekday per weekOnsite role in East Boston
