Develop personalized care plans that align with clients??? goals while coordinating services such as home-delivered meals, congregate dining programs, food bank access, nutritional counseling, and financial benefits programs related to food access.
Provide crisis intervention and support to stabilize older adults experiencing urgent nutritional deficiencies or acute food access issues.
Utilize empathetic and client-centered communication techniques to foster engagement and encourage proactive self-management of health and dietary situations.
Supervise and mentor junior team members or interns, ensuring quality service delivery tailored to the nutritional needs of the aging population.
Collaborate with community partners on programs like SNAP (Food Stamps), Medicare/Medicaid, Area Agency on Aging meal services, and specialized dietary services to streamline client access.
Maintain detailed case records, progress notes, and service documentation in compliance with organizational policies and confidentiality standards, particularly for vulnerable adults.
Working hours: 8:00 AM - 5:00 PM
