**Position Summary**
This is a work from home position with 50-75% travel to meet members face to face. Candidate must live in South Cook or Will County IL particularly in zip codes 60477, 60487, 60449, 60423, or 60443.
Standard business hours: Monday - Friday.
This role provides comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues.
This includes:
-Determines medical necessity/ appropriateness
-Facilitates optimal outcomes
-Identifies and follow through with continuous quality/ compliance opportunities. May also include identification of aberrance's and initiation of corrective action
-Educates/ empowers customers to ensure compliance, satisfaction and promote patient advocacy
-Optimize total costs Implementation and evaluation of policy based on usage and program directives
-Educate/empower colleagues at all levels to enable decision making at most appropriate level
**Required Qualifications**
Minimum 2 years of experience in behavioral health, social services or human services field.
-Minimum 2 years of case management experience.
-Must reside in in South Cook County or Will County IL or Surrounding Areas
-Willing and able to travel using your own vehicle up to 50- 75% of the time to meet members face to face in South Cook County and surrounding areas.
-Reliable Transportation required, eligible for mileage reimbursement as per company policy.
-Valid Illinois Driver’s License
**Preferred Qualifications**
-Discharge planning experience preferred
-Managed Care experience preferred
-Microsoft Office experience preferred, specifically in Excel
**Education**
-Bachelor's degree or Master level , with either degree being in behavioral health or human services required (psychology, social work, marriage and family therapy, counseling)

**Pay Range**
The typical pay range for this role is:
$21.10 - $44.99
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
