Case Management Associate

Position Summary

This position is primarily responsible for facilitating effective communication among caregivers in support of patient advocacy. The role requires close collaboration with the care management team, as well as the clinics and clinicians they support. The individual must be highly accessible, organized, and responsive to meet patient and team needs.

Seeking an experienced Medical Assistant

Essential Job FunctionsAdministrative & Care Coordination Support

Provides administrative and operational support to the case manager and clinic team, including:

Reviewing daily clinic schedules to identify high-cost patientsParticipating in daily clinic morning huddlesParticipating in weekly Utilization Management (UM) and Care Management (CM) roundsPerforming task-driven patient outreach according to the plan of careScheduling follow-up appointments after Emergency Department (ED) visitsSending letters and reminders to patients who are not established or overdue for visitsFollowing up with internal teams to ensure completion of pending tasksPatient Outreach & Case Management SupportProvides Tier Two patient outreach for short-term case managementConducts “tuck-in” calls for real-time rising-risk patientsAssists case managers and clinic staff with daily outreach related to specific care tasksConfirms:Medications are being taken as prescribedPatients attended specialist appointments and updates care goals accordinglyPatients have received Durable Medical Equipment (DME), Home Health Care (HHC), or other resources outlined in the plan of careCare Transitions & DocumentationFollows up with patients regarding completion of referrals, DME, and specialist visitsAssists with required paperwork from hospitals, Skilled Nursing Facilities (SNFs), Long-Term Acute Care (LTAC) facilities, and home health agenciesMinimum Required Education, Experience & SkillsCollege degree or completion of college coursework in healthcare, care management, or a related fieldPrior experience as a Medical Assistant, health coach, or in care management preferredPreferred Education, Experience & SkillsStrong interpersonal and communication skillsAbility to maintain consistent communication with multidisciplinary teamsAbility to multitask in a fast-paced environmentBilingual in English and Spanish preferred, but not required

 

 

Job Type & Location

This is a Contract to Hire position based out of Seminole, FL.

Pay and Benefits

The pay range for this position is $18.00 - $20.00/hr.
