Care Team Coordinator
Overview Helio Healthis on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. We’re currently seeking a skilled and compassionateCare Team Coordinatorto join our team and support the professional growth, accountability, and clinical integrity of our behavioral health team and the program’s day to day operations. The Care Team Coordinator will work on aFull-Timebasis out of Helio Health’sOpen Access ProgramMonday–Friday, 8:00am–4:30pm. In addition to Helio Health’s comprehensive benefits package, this position is eligible for aSign-On incentiveof$8,000 depending on certification or licensure! Pay: $29.00/hr. to $34.05/hr. Responsibilities Key Responsibilities: + Supervision & Staff Development + Provides supervision of program staff, volunteers and student interns. + Continuous staff performance evaluation and supervision. + Quality Improvement + Recommends program standards and measurements for continuous quality improvement. + Monitors team/program performance indicators. + Manages program resolution and makes suggestions for process + Team & Program Support + Assist with the development and delivery of Agency and/or Clinic Policy and Protocol. + Ensure collaboration across the Multi-Disciplinary Team to ensure the quality of all services meet CCBHC, OMH, OASAS and Agency Standards. + Assures program record documentation compliance with agency and regulatory standards. + Provides Direct care services, may carry a caseload. Qualifications + Advanced or Master CASAC or Qualified Health Professional with NYS Licensure required (LMSW, LMHC, LCSW, LMFT, LCAT) + Experience: Minimum three years of clinical experience in an appropriate program setting. + Minimum of Bachelor's Degree.
