UMC is currently seeking a Care Coordinator LPN or RN for The Shores, our Senior Care Community located in Ocean City, NJ within picturesque Cape May County. Requirements for a Care Coordinator: + Bachelor's degree or higher in health-related or behavioral science field with 1 year experience with older adults in a community setting + One year experience in case management marketing + Knowledge of Microsoft Office + Current NJ License Practical Nurse (LPN) or Registered Nurse (RN) + Case manager certification Responsibilities for a Care Coordinator: + Gathers and evaluates all referral information and reviews information received and/or assesses potential residents for admission. Makes admission decisions and/or works with nursing on admission decision for healthcare. + Builds and maintains referral network, conducting onsite visits as needed. + Facilitate the admission process into healthcare and ensures paperwork is signed and processed in a timely manner. + Verify Medicare coverage and/or other benefits. + Participates in care conferences and other meetings as required. + Follows residents to home post discharge to ensure they are okay; completes medication reconciliation; psychosocial assessment. Involved with post-acute discharge at home. + Ensures departmental reports are completed in a timely manner.
