This Behavioral Health Care Coordinator role offers a meaningful opportunity to support individuals of all ages by helping them navigate and coordinate their behavioral health care across internal teams and community providers. You will play a key role in connecting patients to the right services, improving continuity of care, and making a real impact on outcomes.The position is ideal for someone who enjoys a mix of independent work and team collaboration, is highly organized, and takes pride in delivering thoughtful, patient centered customer service. You will work closely with a multidisciplinary care team while also managing your own caseload and daily priorities.
This is a full time, Monday through Friday position with a predictable schedule and full benefits, offering great work life balance along with the chance to build valuable experience in behavioral health care coordination.
Summary
The care coordinator is responsible for providing direct coordination of services to patients and families as directed by the comprehensive assessment, initial service plan and/or treatment plan in support of the treatment team.Job Description
Assists the Behavioral Health Clinic with tracking, referrals, coordination of services and general non-counseling roles of case management. Utilizes all forms of communication to connect with patients and referrals. Facilitates referrals to mental health services as identified in assessment or treatment plan. Documents services provided in electronic filing system. Assists with referrals for economic assistance, social services, housing resources and educational services. Supports life skills advocacy, treatment follow up, disease management and educational services, including referral and linkages to long-term services and supports. Connects patients to community resources to assist in reaching successful outcomes. Tracks and monitors each patients care throughout their involvement with our clinic.Qualifications
A minimum of two years of college in a healthcare related field is required; Bachelor’s degree is preferred.Preferred qualifications include a minimum of two years’ experience, preferably working within a behavioral health clinic or medical facility.
CPR certification and AED certification required. In applicable locations, must have a valid driver’s license.
