Behavioral Case Manager II- Licensed/Certified Behavioral Health Role
Locations: The selected candidate must reside in the state of Virginia.
Field: This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

The Behavioral Case Manager II is responsible for performing is rase management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.
How you will make an impact:
Provide case management services to individuals with serious mental illness (SMI), including those with co‑occurring substance use disorders and/or significant physical health conditions.Deliver services primarily through telephonic outreach, with the possibility of in‑person/face‑to‑face visits when clinically necessary.Conduct regular outreach to assigned members to build rapport, assess needs, link to needed services/resources and provide short-term support (up to 6 months).Support members by helping them navigate urgent and non-urgent needs and connecting them to appropriate resources. Responds to more complex cases and account specific requests.Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.Conducts assessments to identify individual needs and develops specific care plan to address objectives and goals as identified during assessment.Monitors and evaluates effectiveness of care plan and modifies plan as needed.Supports member access to appropriate quality and cost-effective care.Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.Serves as a resource to other BH Case Mgrs.Participates in cross-functional teams projects and initiatives.Minimum Requirements:
Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing, and minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background. Current active unrestricted license such as RN LCSW LMHC LICSW LPC (as allowed by applicable state laws) LMFT LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders.Preferred Skills, Capabilities and Experiences:
Experience in health coaching and motivational interviewing techniques preferred.Knowledge of behavioral health systems of care in Virginia strongly preferred.Strong Written and verbal documentation skills strongly preferred.Intermediate to advanced Microsoft office usage strongly preferred.If this job is assigned to any Government Business Division entity, the applicant and incumbent fall under a 'sensitive position' work designation and may be subject to additional requirements beyond those associates outside Government Business Divisions.
