Associate Manager, Transitional Care Operations

Role Description:

Oak Street Health takes a team-based approach to providing outstanding patient care. The Manager, Transitional Care Operations at Oak Street Health will primarily be responsible for managing Transitional Care Coordinator Supervisors. Transitional Care Coordinator Supervisors provide day-to-day oversight and support to the Transitional Care Coordinator (TCC) team. TCCs are an integral part of the team who help track patients across hospital and post-acute settings and help them get connected back to primary care. 

This remote role will supervise TCCs supervisors, support operational initiatives, drive their team towards achieving operational and clinical outcomes measured via a variety of metrics, and generally ensure the team is running smoothly on a daily basis. 

This includes but is not limited to: onboarding new TCCs, obtaining EMR access, assisting with workflow development, collaborating with Transitional Care RNs, Practice Managers, hospitals, community providers, and care teams, and ensuring the TCC team is exceeding expectations. We also expect you to be highly flexible, covering additional responsibilities, as needed. This will primarily be a remote work position.

Core Responsibilities:

Staff Management

Serve as Transitional Care Coordinator position owner

Coach TCC Supervisors and recommend methods to improve TCC performance 

Hire, train, supervise, coach, mentor and manage team 

Audit TCC Supervisors and TCC Panels

Transitions Operations

Ensure Transitions operations are running smoothly and safely on a daily basis, including ensuring there is adequate staffing each day, arranging coverage for any call-offs or scheduled PTO and ensuring that all Standard Operating Procedures are followed

Support team in identifying, designing, and implementing process improvements

Lead the implementation of operational initiatives, including training the team on new initiatives and workflows

Track resource issues and develops solutions

Partner with Sr. Associate, Population Health to develop and refine reporting needs to track and report on operational processes and productivity

Leadership

Partner with programmatic leadership to lead the Transitions operations and make collaborative decisions that support the best interests of the patients and team

Provide an overview of the role at leadership meetings (such as at PM, PHD, RDO Huddles and onboarding)

Other duties, as assigned

Proficient PC skills

College degree preferred, high school diploma required

People management experience

US work authorization

Pay Range

The typical pay range for this role is:

$46,988.00 - $112,200.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
