Position Summary

Assist Program Director/ Senior Program Coordinator in the daily operation of the program; This includes but not limited to supervision of all staff.

Essential Responsibilities

Monitor and review residents progress notes, service plans, collateral/case notes and give constructive feedback to subordinates

Ensure program and case records are in compliance with regulatory and agency guidelines. Interface and maintain written communication with other service providers and agency support staff.

Monitor monthly billable services via electronic health records and other means of verification( Apt. Program, Horizon and Renaissance ONLY)

Responsible for maintenance and updating of all resident files on a monthly basis

Attend case conferences and provide feedback to clinicians and other external support entities regarding resident’s progress.

Monitor and ensure Medication Management protocols are being completed based on agency and regulatory guidelines.

Responsible for crisis intervention requiring 24 hours on call availability.

Screen candidates for residential placement - this may require attendance at hospital or current placement (i.e. shelter).

Assist supervisor in planning and coordinating recreational/social activities. At times this may require escorting residents to these activities and/or participate in moderate physical activities.

Required to complete walk-through of facility at least twice per day

Responsible for ensuring staff is appropriately monitoring the facility on an on-going basis

Provide training to all staff on an on-going basis starting with new staff orientation.

Complete and coordinate weekly staff schedule

Act as liaison between outside service providers and the community residence. This requires both telephone and site visits to all boroughs.

Attend all scheduled meetings.

Must be able to participate in scheduled meetings/trainings at various sites, at varying times and transport self by car, in a timely fashion to and from destination. Must demonstrate the ability to complete/pass mandated trainings and ability to perform the techniques taught. (i.e. Cultural Competency, CPR and First Aid.)

Participate in weekly supervision with Program Director.

Complete chore schedule and menu planning ( Apartment Program and CR’s ONLY)

Assist with completing bi-weekly payroll in the absence of the supervisor.

Review incident reports and assist Program Director to conduct investigations.

Provide weekly supervision to assigned staff; provide professional direction and technical assistance and delegate tasks.

Bachelor’s Degree in Social Services preferred. A minimum of a high school diploma. Basic computer literacy. Three years working in human services required. Experience working with persons coping with mental health conditions required. Supervisory experience preferred. New York State driver’s license and access to a vehicle required and two years of driving experience required.
