Find a fulfilling career and make a difference with UnityPoint Health. **Responsibilities** + Perform waived laboratory tests, schedule appointments, and document according to established guidelines provided in the Coumadin (Warfarin) therapy guidelines. + Adjust medication dosage based on INR lab results + Assist anti-coagulated patients both in office and via telephone management to assist maintaining a safe and therapeutic dosing regimen of Warfarin. + Work with multi-disciplinaries including Home Health, Long-Term Care, and Skilled Care facilities. + Manage office responsibilities which include documentation, records, and patient outreach. + Accurately administer medications orally, IM, sub-q and IV. + Coordinate the transfer of patients to hospitals. **Qualifications** + **Valid and current RN licensure in the state of Iowa** + Registered Nurse (RN) – Iowa Board of Nursing + Prior experience in a clinical or outpatient setting preferred + Familiarity with EPIC a plus!
