Advanced Practice Provider (APP/NP/PA) Advanced Care Planning
The Advanced Practice Provider (APP) will launch and lead a dedicated Advance Care Planning (ACP) consultation service within UVA Medical Center and affiliated with UVA School of Medicine's Division of Hospital Medicine. The service will initially focus on Cardiology inpatients, with an anticipated expansion to Hospital Medicine, other inpatient populations, and, ultimately, into ambulatory settings. The incumbent APP will work closely with the Health System's Director for Advanced Care Planning on programmatic development and growth, affiliated physician staff, and divisional APP leadership related to operational role development. The APP will provide clinical and operational leadership which will include expanding the service to include additional ACP consultants as necessary. The APP will function with a high degree of clinical autonomy in this novel consultative role, collaborating with primary and interdisciplinary teams to identify, prioritize, and address ACP needs across the care continuum. Preferred Qualifications: * Demonstrated ability to practice autonomously in a consultative role, including clinical judgment, patient prioritization, and ability to collaborate effectively. * Formal training as an Advanced Steps Respecting Choices® ACP Facilitator preferred, with documented experience successfully applying the model in clinical practice (in lieu of existing experience, training will be provided) * Completion of Ariadne Labs Serious Illness Conversation training preferred, with demonstrated proficiency integrating the framework into real-world patient and family discussions (in lieu of existing experience, training will be provided) * Strong knowledge of advance care planning documents and medical orders, including advance directives, DDNR, and POLST, and the legal and ethical principles governing their use. * Expertise in serious illness communication, shared decision-making, and values-based goal setting with patients and families across diverse clinical and cultural contexts. I. Professional UVA Health is enthusiastically starting an APP-led Advanced Care Planning Consult Service. The APP will assess patients based on clinical presentation, disease trajectory, and documented or unmet ACP needs to determine appropriateness and timing for ACP consultation. Using evidence-based communication frameworks, the APP will initiate and facilitate Respecting Choices® Advance Care Planning and Serious Illness Conversations, ensuring that patient values, goals, and preferences are elicited, clarified, and documented in a manner that meaningfully informs current and future care. In the early stages of this program, this program will focus on subsets within the inpatient population. As the program matures, the APP's scope may also broaden to serve ambulatory clinics. The successful candidate will join UVA's Center for Advanced Practice, the system's professional home for APPs, and work closely with the Director of Advanced Care Planning. II. Clinical The APP will evaluate the need for, facilitate completion of, and ensure appropriate documentation of ACP documents, including advance directives, Durable DNR (DDNR) and POLST orders, in compliance with state statutes, regulatory requirements and institutional policies. The APP will be responsible for accurate and timely ACP documentation in the electronic health record to ensure reliability, visibility and accessibility as well as clinical relevance and actionability across care settings. III. Administrative In addition to direct clinical care, the APP will collect and analyze consult-level and program-level data to identify ACP opportunities, evaluate service effectiveness, and demonstrate alignment with institutional priorities. These outcomes include, but are not limited to, 30-day readmission and mortality metrics, patient and family experience measures, appropriate patient progression, avoiding ICU level of care that is not in alignment with patient-defined goals of care, timely hospice referral and enrollment, and clinician satisfaction. The APP will use these data to inform continuous improvement, support scalability of the service, and ensure that care delivered is consistent with known and documented patient preferences. IV. Leadership Serve as a clinical leader working across disciplines, modeling professionalism, accountability, and patient-centered care. Also, supporting a collaborative, respectful culture across physicians, APPs, nurses, trainees, and ancillary staff; and, in a first-of-its-kind role, setting a high standard for collaboration, service, and patient-centered care. MINIMUM REQUIREMENTS Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree. Experience: Department and unit specific. Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease. The starting base rate for this role is $116,123.00 annually.
