$35.50 - $53.25The Admissions Coordinator is responsible for overseeing the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director.
Job Requirements:
Bachelor's degree in Nursing, Physical Therapy, Occupational Therapy, or Speech Therapy with applicable state licenseCPR certificationMOAB (Management of Aggressive Behavior) certification (can be obtained after hire)Five years' clinical experience in a hospital or rehabilitation setting preferredPrior case management experience preferredKnowledge of Inpatient Rehabilitation Facilities regulatory requirements preferredKey Responsibilities:
Completes the pre-admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources withing twenty-four hours Monday through Friday and in accordance with Commission on Accreditation of Rehabilitation Facilities (CARF) and CMS guidelines - Assessments include a review of the medical record, family interviews, and consultation with unit leadership and the rehabilitation physician to determine appropriateness of admissionDetermines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitationMakes recommendations for inpatient rehabilitation or other appropriate post-acute care settings and communicates recommendations to referral sources in a timely mannerEnsures appropriate pre-admission documentation is completeFacilitates daily admissions meeting with unit leadership and the rehabilitation physician to discuss potential admissions to the unit and any barriers that might exist including medical, clinical, financial, or social barriersEnsures accurate collection of referral, denial, and admission's data through maintenance of an excel data fileProvides for a pre-admission orientation to the unit including the provision of written materials including the Inpatient Rehabilitation Center brochure and written disclosure statementSeeks pre-authorization from insurance companies and communicates availability of rehab benefits to prospective admissionsWorks with financial counselors to assist our uninsured with potential access to the rehab level of careEnsures accurate collection of data related to the insurance authorization process for potential admissions through maintenance of an excel data fileCollaborates with the physician and interdisciplinary team to develop and coordinate the patient plan of care, including assisting with the overall goal for the rehab. StayParticipates in Team Conferences as neededKeeps payer sources informed of patient progress and provides follow-up information for utilization review and authorization of additional rehab days as neededWorks with referral sources, physicians, and the interdisciplinary team to identify opportunities to improve the efficiency/effectiveness of the admission's process, improve overall communication, and improve patient outcomesProvides accurate, thorough, and timely information regarding the clinical and financial status of new admissions to the SWCM and other interdisciplinary team members to ensure a seamless transition to the unitProvides ongoing education to referral sources and staff related to admission criteria and regulatory changes that affect the rehab level of carePerforms other duties as assigned
